Office of Insurance Regulation – Update of Forms Worksheets

To Industry Members,

The Office has recently completed a revision to the forms worksheets for major medical and dental products that are compliant with the Affordable Care Act (ACA) standards for products issued on or after January 1, 2014.  These worksheets are intended as guidance for companies to comply with state law, and federal law.  [The Office does review forms for federal compliance on a voluntary basis as part of our Collaborative Enforcement Arrangement.]

In the past, some companies have found it useful to download, complete, and upload these worksheets as part of their filing (including page numbers for specific form provisions intended to comply with different state and federal laws) to help expedite the form filing process. We look forward to working with you to assist you with the form approval process to meet both state and federal requirements and deadlines.
